My Mission

My mission is to provide access to the highest level of OT service and care tailored specifically to you or your loved one’s needs.

The name Achieve It OT embodies my vision - to help you achieve what your [it] is. Whether that be achieve independence, function, safety, skills, wellbeing or any other goal you have.

My priority is to provide the highest level of support, innovation and individualised therapy and services to meet your needs.

Meet The OT

Nerae

Nerae (pronounced ner-ray) the face and Occupational Therapist (OT) behind Achieve It OT!

With over 12 years experience working across regional and rural areas of NSW, Victoria and Queensland - I know first hand the challenges faced and strength of people living regionally.

As an OT I take pride in ensuring sessions are based on what you want to achieve and how that fits in with your life or home environment. No two people are the same and that is why I believe in focusing on what truly matters to you or your loved one.

On the weekends you will find me spending time with my 2 x kids and husband, pottering around in the garden or catching up with friends. I love a good cup of coffee, good food and good company!

Who we support

Achieve It OT is an adult community service specialising in physical, cognitive and neurological OT from 17 years through to 99 & beyond.

We are proudly accepting referrals for self and plan managed participants under National Disability Insurance Scheme (NDIS), Department of Veterans’ Affairs (DVA) and private funding schemes.

DVA Rehabilitation Appliance Program

A personalised assessment by an experienced OT. Conducted in your home on the first visit, it provides insight into your current routines, environment and any challenges you want to overcome. Together we discuss and identify your goals as well as a plan to achieve them. All tailored specifically for you.

Functional Capacity Assessment

From minor grab rails, to ramps, complex bathroom modifications and everything in between. Your OT will thoroughly consider your home, the space available and how you use it to recommend individualised home modification options available to meet your needs.

Also known as equipment and aids. Prescription includes a thorough assessment of your current challenges, home and goals to ensure it will fit into your life and meet your needs. Some items will also be trialled to ensure it is just right for you. Trials can be done in a showroom or at home.

The Rehabilitation Appliances Program (RAP) provides aids, equipment and modifications to help Gold & White card holders live safely and independently. Your OT will complete thorough assessment and identify what aids, appliances or home modifications are required to achieve this.

A GP D0904 referral is required to access this service.

An in depth assessment and report focused on identifying your strengths and support needs to overcome challenges you may be having.

The focus is to support and advocate for you, helping to communicate with the NDIS what you need day-to-day to work towards & achieve your goals.

A program completed with the OT focused on day to day skills in laundry, cleaning & home management, meal preparation (including shopping), basic personal hygiene (including morning routine) and personal safety (emergency procedures).

Sessions are completed in the comfort of your home and are individualised to your specific needs.
